A 52-year-old Italian woman presents with three days of worsening breathlessness. Her last chemotherapy was one week ago and she has a portocath in situ.

On arrival: BP 130/60 mmHg, PR 110/min, RR 28/min, temperature 37.8 degrees C, oxygen saturation 90% on room air. Her chest X-ray is shown below.
