Adequate spontaneous ventilation	Appropriate tidal volume, ETCO2 or ABG, and no concerning chest findings.
Adequate oxygenation	PaO2 above 60 on FiO2 below 0.40 or otherwise low oxygen requirement.
Cardiovascular stability	Stable rhythm and observations, with absent or low-dose vasopressors.
Adequate conscious state	Sustained eye opening and response to commands without continuous sedation.
Adequate strength	Neuromuscular blockade worn off; can lift head or sustain limb movement.
Acceptable acid-base state	No significant acidosis.
