In a rural ED you receive ambulance pre-notification of a 24-year-old man involved in a motorcycle crash with right abdominal and pelvic injuries. ETA is 10 minutes. On arrival he remains shocked despite 2 litres of normal saline and has severe right lower abdominal and pelvic pain. eFAST is negative and CXR is normal. Pelvic X-ray is shown below.
