You are the director of a large teaching hospital ED. Your Nurse Unit Manager tells you that a nurse was asked by a senior registrar to countersign an amended adrenaline order.
The registrar reports that after giving 1 mg IM adrenaline for anaphylaxis, they realised the guideline dose was 0.5 mg IM. The patient developed brief chest pain, hypertension and tachycardia that settled after 5 minutes.
The registrar altered the medication chart so the dose appeared to read 0.5 mg and asked the nurse to countersign the changed order.
