He then becomes unresponsive with BP 65/40 mmHg, HR 18/min, SaO2 90% with a poor trace and RR 10/min. Blood results include sodium 142, potassium 5.1, urea 8.2, creatinine 98, glucose 6.8 and high-sensitivity troponin 18. What is the likely cause of the sudden deterioration?
