A prehospital alert reports an 86-year-old nursing home resident with acute respiratory distress who has achieved ROSC after arresting in the ambulance.

She arrives intubated with GCS 3. Her daughter later presents distressed with an advance care directive stating that her mother was not for CPR, ventilation, ICU care or hospital transfer.

After confirming the directive and discussing with family, a decision is made to extubate her in the ED.
