SAQ 1166 – Quad-bike rollover with pelvic trauma

Ambulance pre-notifies your tertiary trauma centre of a 42-year-old man after an unwitnessed quad-bike rollover on a farm. He has a scalp wound, bilateral rib fractures and a pelvic injury. His RR is 22/min, SpO2 97% on high-flow oxygen, HR 125/min, BP 85/40 and GCS 14 after 1.5 L Hartmann's.

On arrival his airway is patent with a hard collar in place. He has bilateral rib tenderness, remains clinically shocked at BP 80/35 and pulse 130, GCS 14 with equal 3 mm pupils, temperature 34 degrees C and glucose 8.0 mmol/L. Further assessment shows an oozing scalp laceration, open right tibial fracture, negative eFAST, right rib fractures 5-7 on CXR and multiple right pelvic fractures extending into the acetabulum and pubic rami.
