A 31-year-old man is brought by ambulance with dyspnoea and a history of asthma. RR is 38/min, SaO2 96% on Hudson mask at 8 L/min, HR 140/min, BP 140/78 and temperature 36 degrees C. GCS is 14 (E4, V4 confused, M6). VBG: pH 7.33, pCO2 39.4, HCO3 20.3 and lactate 4.3.
