An 89-year-old woman presents with generalised weakness, nausea and diarrhoea. She fell 3 days earlier, sustained a humeral fracture and was discharged for outpatient orthopaedic follow-up. She appears dehydrated, with BP 140/80 mmHg, pulse 80 regular and she is afebrile. Her venous blood gas is shown.
