A 3-year-old boy is brought to the ED from day care after eating nuts. He has an urticarial rash and wheeze, with initially normal vital signs.
He then deteriorates. He is alert, pulse 180/min, blood pressure 65/40 mmHg, respiratory rate 40/min, oxygen saturation 90% on room air, with worsening wheeze and pallor.
After treatment he improves, does not need airway intervention, and is observed overnight.
