SAQ 1982 – Pulmonary embolism imaging and treatment in pregnancy

A 30-year-old woman who is 30 weeks pregnant with her first pregnancy is brought by ambulance with severe shortness of breath and palpitations.

She has mild distress, SpO2 92% on room air, pulse 110 and BP 100/70. Her right leg is swollen with a tender calf. PE is suspected and bedside echocardiography is attempted, but images are non-diagnostic. Wells score is greater than 7.

A segmental PE in the right lobe is diagnosed, without RV strain, and she remains clinically stable.
