SAQ 1989 – Shocked older patient with wide complex tachycardia

A 70-year-old Caucasian man presents unwell and shocked. He is conscious and conversational but pallid, cold and sweaty, with a weak radial pulse.

His BP is 85/50, SpO2 95% on room air and temperature 37.8 degrees C. A urinary catheter has drained less than 50 mL of cloudy urine over 24 hours.

The monitor and 12-lead ECG show a wide complex tachycardia.
