SAQ 1030 – Anaesthetics – severe facial trauma requiring emergency airway plan

A 25-year-old woman has been kicked in the face by a horse and arrives at a rural base hospital unconscious with extensive facial trauma and partial airway obstruction.

She is tachycardic, hypotensive, GCS 3 and hypoxic. Oral intubation is not possible because of deranged anatomy, tongue swelling, blood and debris.

Discuss cannula versus surgical cricothyroidotomy in this situation.
