A 50-year-old man is brought by ambulance to a rural ED on a hot, humid spring day. His wife, a trained nurse, found him unconscious in an agricultural field beside a tractor. His GCS has gradually improved and he is now alert but confused.

Vital signs are temperature 37.4 degrees C, HR 120/min, BP 100/70, RR 18/min, SpO2 96% on room air and GCS 14.
