You are the director of your department and receive a call from an angry patient. He presented with ankle pain, was told he only had a sprain and to take paracetamol, then later learned from his GP that he had a fracture requiring surgery. Review shows the prior X-ray had an obvious Weber B fibular fracture with a report recommending urgent orthopaedic review, and there is no evidence he was uncooperative.
