A 25-year-old woman at 18 weeks gestation presents with severe chest pain and shortness of breath. You are considering pulmonary embolism. Before definitive testing she becomes critically unwell with GCS 15, pulse 145/min, BP 70/45 mmHg, RR 40/min, and oxygen saturation 96% on 15 L oxygen via non-rebreather mask. She is too unstable to leave the resuscitation room, and you decide to perform bedside echocardiography to assess for PE. After assessment you are confident she has a massive pulmonary embolism.
