A 74-year-old woman presents to your tertiary ED with left hemiplegia. You are concerned about ischaemic stroke and arrange urgent CT perfusion imaging. She is a current smoker and has no other known medical history.
After full assessment and review of the CT results, the stroke consultant recommends thrombolysis. The patient has capacity to consent.
A bolus of alteplase is given and a 60-minute infusion is commenced. At 30 minutes, she develops large-volume haematemesis.
