A 2-year-old boy presents to a rural ED with active stridor but is otherwise well.

He is diagnosed with croup, has significant work of breathing, tachypnoea and audible biphasic stridor, with respiratory rate 40/min and oxygen saturations 95% on room air. You have given medical treatment and are preparing for intubation.

He deteriorates, intubation fails, and a sub-glottic airway procedure is required.
