A 75-year-old patient with ischaemic heart disease and pulmonary hypertension is intubated for a COPD exacerbation. After induction and intubation the patient is apnoeic and is hand-ventilated with high pressures. Oxygen saturations are 80% on FiO2 1.0, heart rate is 155 bpm and irregular, and blood pressure is 70/50 mmHg.
