You are assessing a 60-year-old man who has been reluctantly brought to ED. His housemate is concerned he is not his usual self. He has been agitated, has not been sleeping well, and has been talking about hanging himself. He uses regular cannabis and has no other medical history.

On examination he appears withdrawn, has a faint bilateral wheeze, and refuses further examination.
