SAQ 1975 – Unwell diabetic patient with VBG before intubation

A 32-year-old man is brought to your ED by ambulance after several days of lethargy, nausea and feeling generally unwell.

He has type 1 diabetes and uses IV methamphetamine. Initial VBG: pH 7.08, pCO2 33, HCO3 10, sodium 99, potassium 6.5, chloride 57, glucose 119 and lactate 3.6.

His observations are HR 140, RR 16, BP 100/65, oxygen saturations 98% on room air and GCS 8. You decide to intubate him emergently; estimated ideal body weight is 60 kg.
